“Apple Core” Breast nipple reduction in Las Vegas
Stephen W. Gordon, M.D.

Large nipples might be great for nursing an infant, however nipples that are too large can spoil the appearance of otherwise very nice looking breasts. Large nipples can limit and negatively impact clothing options and become a constant source of embarrassment and invite undesirable attention and commentary. Large nipples constantly appear to be erect and can be seen through clothing if a padded bra is not worn to conceal the condition.

Large nipples are not uncommon and quite often are accepted as being normal. For this reason, large nipples are unlike the condition of inverted nipples. Inverted nipples create a much greater embarrassment and prompt women to seek corrective treatment early and specifically for that condition. In contrast, a large nipple reduction is most often done in conjunction with a breast augmentation or some other cosmetic breast surgery.

Large nipples are reduced by first removing a strip of nipple skin from the sides of the cylinder of the nipple. We have now created what I refer to as an “apple core”. I then collapse and carefully suture the cap or tip skin of the nipple down to the base area of the nipple (see diagram 1.). Occasionally, if the nipple is still too broad, I will take a small “pie shaped “wedge of nipple skin out of the cap area to create a smaller and more pointed nipple. There is very little if any loss of nipple sensitivity and complications are rare and very minor should any occur. Sutures are removed in 7 or 8 days. This operation is very effective and is easily performed by itself or combined with other procedures.
